Class Donation Request

Use this form if you have specific product needs for an upcoming class.

Thank you for thinking of Making Memories as you build and design your classes.  We appreciate your business and hope to be able to help you in the best possible way to teach your class and represent Making Memories.

As we do receive many product requests each month, we cannot guarantee all requests will be fulfilled.

All requests must be submitted at least 45 days before desired receive date.

All requests must be submitted online. Requests submitted by fax, e-mail, or phone calls can not be considered. An email response will be sent within 30 days to notify you if your request will be fulfilled or not.

You must upload the image of the class project that you are requesting product.  Requests without an image may be denied.
